Resumen
Los agonistas de dopamina son cada vez más utilizados como terapia de primera línea en quienes son apenas diagnosticados con enfermedad de Parkinson (EP). A medida que la enfermedad progresa, los agonistas de dopamina por sí mismos no controlan completamente los síntomas de la EP, y se hace necesaria terapia dopaminérgica adicional.
Los participantes serán inscritos en el estudio en la visita de selección e inicial en base a su necesidad de un control de síntomas de EP adicional, a pesar de su tratamiento de agonistas de dopamina. Se les entregará rasagilina o un placebo que le cubra con los siguientes objetivos:
Medir la eficacia de rasagilina como tratamiento adjunto a la terapia de agonistas de dopamina en pacientes con EP temprano que no se vean óptimamente tratados con agonistas de dopamina.
Para investigar si rasagilina influencia los efectos adversos de los agonistas de dopamina.
Para caracterizar la eficacia de rasagilina en reducir la discapacidad, incluyendo síntomas no motores.

Equilibrio, Sentido del olor disminuido, Ataques de "congelamiento", Síntomas generales de la EP, Pérdida de habilidades motrices, Cara "enmascarada" (con aspecto de tal), Dificultades en la postura corporal, Rigidez, Problemas para dormir, Lentitud del movimiento (bradiquinesia), Temblores, Movimientos incontrolables (disquinesia), Caminar/Andar

- Menos de seis meses
- 3 visitas en intervalos de 9 semanas

Inclusion Criteria
- Los participantes recibirán una dosis estable de ropinirol o pramipexole vía oral a quienes sus síntomas no sean óptimamente controlados o a quienes el régimen de agonistas de dopamina se le detuvo por intolerancia.
- El tratamiento de agonistas de dopamina debe haber continuado durante al menos 30 días, mas no más de cinco años anteriores a la visita inicial.
- Las mujeres con potencial para procrear deben estar de acuerdo en practicar un método aceptable de control de la natalidad.
- Diagnóstico de enfermedad de Parkinson. Confirmación de la enfermedad en la visita de selección e inicial por la presencia de signos cardinales durante al menos dos años (temblor en reposo, bradicinesia, rigidez), sin otras causas de parkinsonismo conocidas o sospechadas.
- La dosis de agonistas de dopamina deben haber sido estables durante 30 días o más antes de la visita inicial.
- Para los participantes que se encuentren recibiendo amantadina o anticolinérgicos, la dosis debe haber sido estable al menos 30 días antes de la visita de selección.
- Los participantes deben estar deseosos y ser capaces de dar un consentimiento informado por escrito antes de realizar cualquier procedimiento por escrito.
Exclusion Criteria
- Pacientes que recibieron rasagilina o cualquier otro inhibidor de la monoamino oxidasa dentro de los 60 días anteriores a la visita inicial.
- Pacientes que recibieron levodopa durante más de 21 días consecutivos dentro de los 90 días anteriores a la visita inicial.
- Pacientes que experimentan fluctuaciones motoras moderadas a severas.
- Pacientes con daño hepático (un defecto en el hígado, a juicio del investigador).
- Cualquier medicamento en investigación durante los últimos 30 días que preceden a la visita inicial.
- Uso de agonistas de dopamina durante al menos cinco años antes de la visita inicial.
- Pacientes con depresión importante, daño cognitivo significativo, o un trastorno de control de impulsos (ICD, por sus siglas en inglés)
- Mujeres gestantes o lactando, o mujeres que planeen quedar gestando dentro de las siguientes 18 semanas.
- Pacientes con hipertensión (alta presión arterial) no controlada. Pacientes hipertensos cuya presión arterial sea controlada dentro de los parámetros del estudio serán elegibles.
- Los inhibidores concomitantes de la monoamino oxidasa o medicamentos contraindicados para el uso de inhibidores de la monoamino oxidasa no serán permitidos. Los medicamentos no permitidos incluyen: Meperidina, dextrometorfano, tramadol, hierba de San Juan, metadona, dextropropoxifeno, ciclobenzaprina, cocaína, ciprofloxacina u otro inhibidor del citocromo P450 1A2 (CYP1A2).
