Study of Rasagiline as Add-on to Dopamine Agonists in the Treatment of Parkinson’s Disease

Official Study Title: Double-blind,  Double-blindClinical study design in which neither investigators nor participants know who is receiving the investigational drug and who is receiving a placebo. Placebo  PlaceboAn inactive substance or procedure (often a pill, liquid, or powder) that has no biological effect. In clinical trials, experimental treatments are often compared with placebos to assess the treatment's effectiveness. In some studies, the participants will be assigned to a control group and will receive a placebo instead of an active drug or treatment. Benefit of Rasagiline as an Add on Theapy to Stable Dose of Dopamine  DopamineA "chemical messenger" that regulates movement by assisting in the effective communication (transmission) of electrochemical signals in the brain from one nerve cell (neuron) to another. As dopamine producing cells degenerate with advancing PD, they no longer produce enough to regulate neurons elsewhere in the brain, resulting in a loss of control of movements, leading to symptoms such as slowed movements, tremor, and rigidity. Summary

Dopamine agonists are increasingly used as first-line therapy  TherapyAnother word for “treatment”. for those who are newly diagnosed with Parkinson’s disease (PD). As the disease progresses, dopamine agonists alone do not fully control the symptoms of PD and additional dopaminergic therapy is needed.

Participants will be enrolled in the study at the screening/  ScreeningPeriod of selection of clinical trial participants based on ELIGIBILITY CRITERIA. baseline  BaselinBeginning measurements against which a participant’s progress can be quantified at the end of a study. visit based on need for additional PD symptom control despite dopamine agonist  Dopamine AgonistA chemical or drug that mimics the role of dopamine in the brain. treatment and will be dispensed rasagiline or matching placebo for the following objectives:

  • To assess the efficacy  EfficacyThe extent to which a specific intervention, procedure, or regimen produces a beneficial result under ideal conditions. of rasagiline as add-on treatment to dopamine agonist therapy in people with early PD not optimally treated on dopamine agonists.
  • To investigate whether rasagiline influences the adverse effects of the dopamine agonists.
  • To characterize the efficacy of rasagiline on reducing disability, including non-motor symptoms.  Non-Motor SymptomsContribute to severe disability and impaired quality of life in advanced Parkinson's disease. Symptoms include anxiety, depression, cognitive mood swings, dementia, constipation, pain, genitourinary problems, sudden drop in blood pressure upon standing (orthostatic hypotension), excessive sweating, and sleep disturbances, sense of smell, vision, memory, weight loss, psychosis, hallucinations, and loss of energy.

Symptoms Addressed: Movement and Non-movement

Balance, Diminished sense of smell, Freeze Attacks,  Freeze AttacksThe sudden loss of ability to move. General PD symptoms, Loss of Motor Skills, Masked Face,  Masked FaceLoss of facial expression and reduced blink rate and eye movement make a person appear to be staring off in the distance, which can be misinterpreted as lack of interest or depression. Postural difficulties, Rigidity,  RigidityA symptom in which muscles feel stiff and display resistance to movement even when another person tries to move the affected part of the body. Sleep disturbances, Slowness of movement (bradykinesia)  BradykinesiaSlowness of movement. , Tremor,  TremorShaking or rhythmic movement, especially in the hands but in other parts of the body. Often occurs when at rest, such as in the RESTING TREMOR. Uncontrollable movements (Dyskinesia)  DyskinesiaTwisting, turning or other abnormal involuntary movement usually involving the arms, legs, trunk, and head. It is most commonly associated with long-term use of levodopa. , Walking/Gait  GaitWay of walking, which is diminished in people with Parkinson’s who tend to shuffle their feet, take too short steps, and not swing one or both arms.

Time Commitment

  • Less than six months
  • 3 visits at 9 week intervals

Eligibility

  • Minimum Age: 30
  • Gender(s) Accepted: Either

Inclusion Criteria

  • Participants receiving a stable dose of oral ropinirole or pramipexole whose symptoms are not optimally controlled or whose oral dopamine agonist treatment regimen was stopped due to intolerability.
  • Dopamine agonist treatment must have been ongoing for 30 days or longer, but no longer than five years preceding the baseline visit.
  • Women of childbearing potential must agree to practice an acceptable method of birth control.
  • Diagnosis of Parkinson’s disease. Disease confirmed at screening/baseline by the presence of at least two of the cardinal signs (resting tremor,  Resting TremorA tremor of a limb that increases when the limb is at rest. bradykinesia, rigidity), without other known or suspected cause of Parkinsonism.
  • Dopamine agonist dose must be stable for 30 days or longer preceding the baseline visit.
  • For participants who are receiving amantadine or anticholinergics, the dose must have been stable for 30 days or longer prior to screening.
  •  Participants must be willing and able to give written informed consent  Informed ConsentThe process of providing information to potential study participants to help them decide whether or not to enroll in a specific clinical trial. prior to performing any study procedures.

 

Exclusion Criteria

  • People receiving rasagiline or any other MAO inhibitor within 60 days preceding the baseline visit.
  • People receiving levodopa  LevodopaA compound that is converted into dopamine (the brain chemical which is deficient in people with Parkinson's) in the brain. for more than 21 consecutive days within 90 days prior to the baseline visit.
  • People experiencing moderate to severe motor fluctuations.
  • People with hepatic impairment (a liver defect, in the investigator’s judgment).
  • Any investigational medications within 30 days preceding the baseline visit.
  • Dopamine agonist use at least 5 years prior to baseline,
  • People with major depression, significant cognitive  CognitiveRelated to mental activities having to do with perception, memory, judgment, and reasoning. impairment or an impulse control disorder (ICD)
  • Pregnant or lactating women or women planning to become pregnant within the next 18 weeks.
  • People with uncontrolled hypertension (high blood pressure). Hypertensive people whose BP is controlled with study parameters are eligible.
  • Concomitant MAO inhibitors or medications contraindicated for use with MAO inhibitors are not allowed. Disallowed medications include: Meperidine, Dextromethorphan, Tramadol, St. John’s wort, Methadone, Propoxyphene, Cyclobenzaprine, Cocaine, Ciprofloxacin or other CYP1A2 inhibitors.
